Nanami first lays eyes on Utena in episode 3 ("On the Night of the Ball"). At first, Nanami is rather indifferent to her, shrugging off Utena as "the girl who came with Himemiya". But just as Anthy's humiliation began, beautiful and ever loyal Utena swept into view, stealing the spotlight and saving Anthy. From that point on, Utena became a contender for Touga's affection. Learning that it was Touga who sent Utena her dress, Nanami began to fiercely dislike Utena for trying to take Touga away from her. Ultimately, that's how Nanami views Utena -- as a dangerous rival for her older brother's attention. Nanami holds an undisguised contempt towards Utena, from plotting her constant humiliation to telling her off outright at Touga's birthday party, and challenging her to a duel.
Of course, Utena also thinks Nanami's a little eccentric, as Utena has been present for most of her weirder episodes. Most of the time, Utena tends to shrug off Nanami's erratic behaviour, and isn't afraid to tell Nanami just how silly she's being (which is more than I can say for most of the students at Ohtori). During episode 16 ("The Cowbell of Happiness"), Utena is the only one who was willing to tell Nanami about the fact that Nanami was wearing a cowbell. Utena is also a bit of a scapegoat to Nanami -- Utena has been accused of everything from having bad fashion sense, to seducing Touga, to trying to kill Nanami. Of course, Utena denies it all -- but really, does Nanami listen to anyone?
The only real thing that ties Nanami and Utena together is Touga. Nanami is insanely protective of her brother, and Utena is the one person whom Touga is genuinely interested in. No matter how much Utena denies anything is going on between Touga and her, Nanami still holds her in contempt for even trying to come near her brother. At one point, Nanami gets so angry that she strikes Utena's flowers, calling her a shameful girl, for trying to seduce Touga, and this leads up to the duel. Of course, Utena beats Nanami hands down at the duel arena -- but even though, Nanami didn't care. All she cared about was Touga, and really, Utena was the only true threat to her bond with Touga...
