Of all of Nanami's relationships, I would say that her friendship with Miki is pretty normal -- they have a pretty casual relationship, as friends. Miki, talented and brilliant is often asked by Nanami to play at her parties, and she admires his intelligence. Nanami knows that Miki has feelings towards Anthy, but for the life of her, she can't understand why. In turn, Miki jokes about her devotion to Touga -- and her relationship with Tsuwabuki. Both Nanami and Miki can be seen some episodes, like in episode 4 ("The Sunny Garden -- Prelude"), she accompanies Miki to Utena and Anthy's dorm to execute her plans to humiliate Anthy. The only thing that really smudges this relationship is Nanami's disbelief that Anthy is Miki's "shining thing". Nanami is so set against Anthy that she refuses to see how anyone can see anything good in her, and doesn't understand Miki's need to find his "shining thing".
What does Miki think of Nanami? I'd say he thinks of her as a regular school chum -- albeit a rather crazy one. And like everyone else (minus Utena) at Ohtori Academy, he is also one of the people who are afraid to tell Nanami when she's doing something outrageously Nanami-like. Of course, it's not so much that Miki is afraid of her, as he is tactful -- and since when does Nanami ever listen?
