It was a bit challenging for me to dig up information on the creators of Serial Experiments Lain (since no one seemed to have more than a line or two on the subject) but here what I've come up with:

Directed by: Ryutaro Nakamura
(Also directed Kino No Tabi, Sakura Taisen, Legend of Crystania)

Written by: Chiaki J. Konaka
(Has also done work with Bubblegum Crisis Tokyo 2040, The Big O, Digimon, Mahou Tsukai Tai, and more)

Original Character Design by: Yoshitoshi Abe
(His artwork can also be found in Niea_7 and Texhnolyze. He is also the creator of the series, Haibane Renmei)

Animation Chracter Design by: Takahiro Kishida
(Has done a lot of animation work: some titles he has contributed to include Haibane Renmei, Macross Zero, Master of Mosquiton, and Tenchi Muyo Movie 1: Tenchi in Love)

Music by: Reichi "Chabo" Nakaido
(Produced and wrote the soundtrack for Serial Experiments Lain, and is a very well-known japanese music artist)
